NOTICE TO FREEZE OR CANCEL
By signing below, I agree that I have read all the terms and conditions in my membership contract, and I understand that my membership will be charged in full 30 days after I submit this notice.
All memberships may be canceled or frozen for any reason upon a written notice of cancellation at least 30 days prior to the first day of your final billing cycle. (For example, if you are scheduled with a periodic billing date on March 14th and you want to cancel or freeze for the month of March, you must provide written notice by February 14).
Your account will continue to be billed at your contractual membership rate until all the conditions for cancellation or membership freeze are satisfied.
A $10 fee will be billed for every month of a membership freeze, on the date agreed upon in your original membership agreement.
